*Retailer/Agent Terms and Condition (signed with date)
*One (1) valid ID (photocopy with picture)
*Smart Money Card Number (photocopy signed by Retailer and CSS/CSME)
*Tax Identification Number (personal TIN)
*Barangay Permit or Business Permit
*Picture and sketch of the store
3. Documents will be processed for accreditation and issuance of agent code.
4. Documents will be verified so be sure they are complete.
5. When validated, agent code will be given for activation.
6. PLDT Channel Sales Team will call the Retailer and ask for the agent code for activation.
7. Commission will be sent via Smart Money.
